Revised COVID-19 Protocols for Our Office:
1. PRE-APPOINTMENT SCREENING. Please print and complete the Patient Advisory and Screening form and bring with you to your appointment OR you may complete the online form when you receive your email or text appointment reminder. Please reschedule your appointment if you have any symptoms of fever, cough, shortness of breath, other flu-like symptoms, or if you have been in contact with someone diagnosed with COVID-19.
2. FACE MASK. Please wear a face mask/covering while you are inside the office. We will let you know when to remove it once we are ready to begin your treatment or procedure.
